Root cause analysis is a structured process aimed at identifying underlying systemic causes, not just quick fixes, using evidence-based practice combining research, clinical expertise, and patient values.<br /><br />The six RCA steps are: defining the problem factually without assumptions; gathering diverse data including logs, policies, and real-time clinical observations; identifying contributing factors; pinpointing root causes; designing and implementing targeted corrective changes; and evaluating results. The Five Whys technique helps delve deeper into causes by repeatedly asking why problems occurred.<br /><br />In a pressure ulcer scenario, contributing factors such as inadequate skin assessments, cushioning issues, and staffing shortages were identified. Root causes like insufficient acuity-based staffing were addressed by system changes rather than relying on weak interventions like training alone. Corrective actions are categorized as strong (system changes), intermediate, or weak (training, reminders), with strong actions preferred to ensure lasting improvements. Sustained monitoring and staff input are crucial for success. APACN provides tools for facilities to apply these RCA processes to improve clinical outcomes.
