Video Summary
The webinar focused on upcoming October 1 ICD-10-CM updates for fiscal year 2027, with an emphasis on long-term care and post-acute care settings. The presenter explained where to find the CMS addendum, conversion table, official guidelines, and alphabetic/tabular list updates, and stressed that facilities should prepare before implementation by reviewing deleted, revised, and new codes, updating manuals and EHR processes, and training staff.

Key coding changes included revisions to excludes notes, code first/use additional instructions, and many new or more specific diagnosis codes. Highlighted updates included secondary oral, laryngeal, and pharyngeal cancer codes; cardiomyopathy and arrhythmia refinements; plantar fasciitis versus plantar fascial fibromatosis; expanded osteomyelitis laterality/site coding; BMI category splits; personal history of C. diff; pelvic abscess reorganization; post-bariatric hypoglycemia; pulmonary mycetoma; inflammatory syndromes; and changes to sinusitis and other infection-related entries.

The presenter repeatedly emphasized that coding must always follow physician or qualified provider documentation, not just EHR suggestions or code crosswalks. Facilities should run diagnosis reports to identify obsolete codes, review clinical records for specificity, query providers only when supported by documentation, and verify MDS, billing, therapy, and PDPM mapping impacts. Returned-to-provider codes were also discussed as a signal to reassess whether the condition truly supports skilled Medicare coverage.

The session concluded with operational advice: update codes yearly, ensure leadership budgets for resources, and keep teams aligned on documentation, coding, and billing workflows.
