Video Summary
The webinar’s experts answered questions on MDS coding, documentation, and clinical practice. They emphasized that community responses should be evidence-based and that experts review posts to provide corrections when needed. Key guidance included: - Respiratory therapy: For ARDs on or after October 1, only skilled respiratory therapy may be counted. Documentation must show medical necessity, the resident’s condition, and why the treatment requires a competent nurse or respiratory therapist. An order alone—or the provider’s credentials—does not make a service skilled. - Weights and interviews: Section K weights must be recorded within 30 days of the ARD; weight-change comparisons are calculated from the date of the recorded weight. When multiple valid BIMS or PHQ interviews occur during the look-back period, use the one closest to the ARD and retain all interviews. - Falls, skin, and isolation: Hybrid fall measures will use MDS and claims data; fall prevention and clear communication with hospitals are important. Nutrition interventions and skin treatments need supporting documentation. Reopened pressure injuries are not considered present on admission if they healed and later reopened. C. diff isolation decisions should be individualized and involve infection prevention staff. - Section GG: If a resident cannot complete a task such as walking the required distance or picking up an object, code it as not attempted with the appropriate reason—not dependent. Coding should reflect what occurred during the assessment period. - Diagnoses and billing: Confirm diagnoses are provider-supported and relevant to skilled care. Modify an MDS only when the facility agrees it contains an error. - Other topics: The experts discussed COVID vaccination conversations, ER versus observation discharge timing, and addressing recurring problems through process improvement—not education alone. The panel encouraged attendees to request more Q&A sessions.
