This presentation addresses F623, the regulatory requirements for notice before transferring or discharging residents from long-term care facilities. Facilities must provide written, understandable notice at least 30 days prior to a facility-initiated transfer or discharge, including the reason, effective date, new location, appeal rights, and contact information for the State Long-Term Care Ombudsman and advocacy agencies for residents with disabilities. Notices must also be sent to the Ombudsman simultaneously. Exceptions allow shorter notice for emergencies or urgent health needs. Resident-initiated discharges do not require Ombudsman notification. Facilities must document reasons for transfers/discharges and maintain records. Proper planning is essential, especially if the facility closes. When cited for noncompliance, leadership must perform root cause analysis, address deficiencies for affected and potential residents, implement systemic corrective actions, provide staff training, and monitor compliance.
