This presentation reviews F-554, which addresses the clinical appropriateness of resident self-administration of medications in long-term care facilities. Per Appendix PP of the SOM Guidelines, residents have the right to self-administer medications if the interdisciplinary team (IDT) determines it is safe and appropriate. The IDT must assess medication appropriateness, residents’ physical and cognitive abilities, understanding of medication instructions, storage safety, and refusal comprehension. Decisions must be documented in medical records and periodically reassessed. If unsafe, alternatives enabling participation should be considered. Medication errors by self-administering residents are not counted as facility errors but may prompt review of staff judgment. Facilities cited for deficiencies must perform root cause analysis, develop corrective plans addressing affected and potential residents, update policies, train staff in assessments and procedures, and implement monitoring plans with audits to ensure compliance. Leadership should ensure systemic changes, continuous training, and thorough documentation to maintain compliance and resident safety.
