Pdf Summary
The 2025 AAPACN update provides an overview of Quality Reporting (QRP), Quality Measures (QM), and Value-Based Purchasing (VBP) programs related to long-term and post-acute care facilities, specifically addressing skilled nursing facilities (SNFs) and long-stay resident populations. The information serves as a guideline and does not constitute legal advice.

Quality Reporting Programs utilize MDS 3.0 data and other sources to track various resident and facility-level measures, reported through platforms like iQIES and publicly on CMS's Care Compare and Five-Star systems.

Short-Stay Quality Measures include rates of antipsychotic medication administration, appropriate vaccination (influenza and pneumococcal), emergency department visits, rehospitalizations, hospitalizations per 1,000 resident days, discharge to community success rates, healthcare-associated infections requiring hospitalization, and Medicare spending per beneficiary. Notably, some of these measures began applying to the programs in fiscal years 2025 and 2027.

Long-Stay Quality Measures track resident outcomes such as falls with injury, pressure ulcers, urinary tract infections, catheter use, incontinence, physical restraint use, increased need for assistance with daily living activities, weight loss, depression symptoms, antipsychotic and antianxiety medication use, behavioral symptoms, and vaccination coverage. Several measures were updated or replaced in late 2023 to improve accuracy and relevance.

Additional measures include staffing-related metrics such as nursing hours per resident day and nurse turnover, as well as vaccination coverage among healthcare personnel, reported through hospitalization and safety networks.

Certain older measures have been retired or replaced due to changes in clinical focus or better measures being available, including self-reported pain measures, improvements in resident function, and some pressure ulcer tracking metrics.

Overall, the document outlines ongoing monitoring and reporting standards designed to improve care quality, health outcomes, and resource use in skilled nursing and long-term care settings, with phased implementation dates through 2027.
